SBAR & Full Handoff Builder

Paste the Copy complete chart output from Clinical Quick Entry Pro. Generate both reports from the same text. No network requests, auto-upload, or stored patient data.

Privacy: Use de-identified classroom/simulation data only. Do not paste patient names, MRNs, birth dates, or other PHI into a public educational website. Always verify the generated handoff against the source chart; never use it as a substitute for your clinical record.

1. Paste clinical documentation

Recognizes Charting Pro's patient summary, 0800/1200/1600 assessments, vital signs, MAR, labs, I/O, devices, and narrative notes. Unrecognized details stay in the source box for review.

2. Report output

Paste your charting above and select Generate both reports.

How it handles missing data

Absent code status, allergies, vitals, clinical events and outstanding tasks are labeled Not documented — verify, not guessed. SBAR is concise for calling a provider; Full Handoff is longer for nurse-to-nurse shift report. The original clinical chart remains the source of truth.

What is SBAR?

Situation: why you're calling. Background: diagnosis and relevant history. Assessment: objective findings and changes. Recommendation: what you need clarified, reviewed, or acted on. The tool does not invent provider orders or claim an intervention was completed.

Local-only formatting tool. Does not diagnose, triage, prescribe, or transfer data to a hospital EHR. Review printouts for patient privacy before distribution.
